Norman N. Monitz MD, Beloved husband of 69 years to Mildred nee Sachs.

Proud father of Robert Monitz, and the late Maris (Jack) Rodgon and the late Dr. Marc Monitz. Happy grandfather of Robert Rodgon, Judah (Erin) Rodgon, Lauren Monitz and Jay Monitz. Adoring great grandfather of Mara and Morris Rodgon. Dear son of the late Morris and Becky Monitz. Beloved son in law of the late Morris and Esther Sachs. Cherished brother of the late Gertrude (The late Max) Erman and Lillian (The late Leo) Friedman. Fond Brother in law of the late Dr. Herbert Sachs and the late Renee. Survived by many nieces and nephews. Past president of FBH, IAFP Board Member, AAFP member, ABFP Charter Member, STFP, University of Illinois Professor of Family Physicians. Former head of St. Joseph Family Practice Education, Former head of Illinois Masonic Medical Center Department of Family Practice, Introduced the first geriatric sport medicine program at the University of Illinois Chicago Circle, Board member of the Illinois Medical Society, Board Member of the Chicago Medical Society, Former president of A.G. Beth Israel Congregation, Past President of Peterson Park B'nai Brith, Silver Circle member of JUF.
